How do I make an Appointment?
Simply call us at 501-663-6455 during our working hours or you can request an appointment online using this form.
Do I need a referral to make an appointment?
You will only need to obtain a referral for a specialist visit with Dr Moore if your insurance requires a referral in order to cover the visit.
What to bring for your initial consultation?
For you initial consultation, you will need to bring the following:
- Current insurance card
- Current drivers’ license
- List of medications
- X-rays or scan results (if applicable)
- Referral (if required by your insurance plan)
We encourage you to come to your initial consultation with a written list of questions to ensure you don’t forget to ask them when you are seeing the doctor.
Who do I contact if I have an emergency and the office is closed?
In the case of a *medical emergency*, please contact the medical exchange at 501-663-6900, call 911 or go to your nearest emergency room. We do not provide prescription refills after normal office hours.
